| dc.description.abstract | El shock es una causa importante de morbilidad y mortalidad en las unidades de cuidados intensivos pediátricos, donde su reconocimiento tardío puede conducir rápidamente al deterioro hemodinámico y a la falla orgánica. Dado que las alteraciones en la perfusión periférica se reflejan en la distribución de la temperatura cutánea, la imagen térmica podría proporcionar un enfoque no invasivo para la detección temprana del shock. Este estudio evalúa el uso de gradientes térmicos periféricos, específicamente la diferencia de temperatura entre el abdomen y el pie, como marcadores fisiológicos no invasivos para mejorar la predicción del choque. La perfusión periférica se refleja en la distribución de la temperatura cutánea, lo que proporciona el fundamento fisiológico para utilizar mediciones termográficas como marcadores indirectos del estado circulatorio. Con base en este fundamento, los gradientes térmicos abdomen–pie y las variables clínicas basales se utilizaron como entradas de varios modelos de aprendizaje automático entrenados para estimar el estado futuro de choque en diferentes horizontes de predicción. El objetivo de predicción se definió como un desenlace binario derivado del índice de choque (SI, por sus siglas en inglés), calculado como la razón entre la frecuencia cardíaca y la presión arterial sistólica. Para desarrollar estos modelos predictivos, se utilizó un conjunto de datos estructurado temporalmente. En la línea de base (t = 0), se registraron las intensidades térmicas extraídas de imágenes termográficas de las regiones del abdomen y del pie, junto con la frecuencia del pulso y la edad del paciente. Estas mediciones térmicas extraídas, y no las imágenes termográficas originales, constituyeron los datos térmicos de entrada. Posteriormente, se obtuvieron mediciones de presión arterial sistólica y frecuencia cardíaca en horizontes temporales futuros de 30 minutos, 3 horas, 6 horas y 12 horas, lo que permitió calcular el índice de choque en cada punto temporal y convertirlo en los correspondientes objetivos binarios de predicción. El desempeño de los modelos se comparó entre las distintas ventanas de predicción, y la importancia de las variables se analizó sistemáticamente mediante valores SHAP para interpretar la contribución de las variables térmicas y clínicas a lo largo del tiempo. También se realizó un estudio de ablación para evaluar el impacto de incluir o excluir características derivadas de la temperatura en los modelos evaluados. En conjunto, los hallazgos respaldan el potencial de combinar gradientes térmicos con signos vitales convencionales para mejorar la identificación temprana y confiable de pacientes pediátricos en riesgo de choque. Este enfoque resalta el valor clínico del monitoreo térmico no invasivo integrado con aprendizaje automático para apoyar la toma de decisiones en cuidados intensivos pediátricos. | |
| dc.description.abstractenglish | Shock is a major cause of morbidity and mortality in pediatric intensive care units, where delayed recognition can rapidly lead to hemodynamic deterioration and organ failure. Since alterations in peripheral perfusion are reflected in skin temperature distribution, thermal imaging may provide a noninvasive approach for early shock detection. This study evaluates the use of peripheral thermal gradients, specifically the temperature difference between the abdomen and the foot, as noninvasive physiological markers to improve shock prediction. Peripheral perfusion is reflected in the distribution of skin temperature, providing the physiological basis for using thermographic measurements as indirect markers of circulatory status. Based on this rationale, abdomen–foot thermal gradients and baseline clinical variables were used as inputs for several machine learning models trained to estimate future shock status at different prediction horizons. The prediction target was defined as a binary outcome derived from the shock index (SI), calculated as the ratio of heart rate to systolic blood pressure. To develop these predictive models, a temporally structured dataset was used. At baseline (t = 0), thermal intensities extracted from thermographic images of the abdomen and foot regions were recorded together with pulse rate and patient age. These extracted thermal measurements, rather than the original thermographic images, constituted the thermal input data. Systolic blood pressure and heart rate were then obtained at future time horizons of 30 minutes, 3 hours, 6 hours, and 12 hours, allowing the shock index to be calculated at each time point and converted into the corresponding binary prediction targets. Model performance was compared across the different prediction windows, and feature importance was systematically analyzed using SHAP values to interpret the contribution of thermal and clinical variables over time. An ablation study was also conducted to assess the impact of including or excluding temperature-derived features in the evaluated models. Overall, the findings support the potential of combining thermal gradients with conventional vital signs to improve the early and reliable identification of pediatric patients at risk of shock. This approach highlights the clinical value of integrating noninvasive thermal monitoring with machine learning to support decision-making in pediatric intensive care. | |
