This work presents a scientometric study and systematic review, conducted under the PRISMA 2020 statement, of the effects of per- and polyfluoroalkyl substances (PFAS) on human health, with emphasis on Colombia. Based on a corpus of 8,221 deduplicated records from Scopus and Web of Science (2015–2026) and a deep-read subset of 41 documents, three objectives were pursued: to characterize global research trends, to establish the relationship between the molecular structure of PFAS and their persistence, bioaccumulation and toxicity, and to assess the feasibility of mitigation technologies for the country. The scientometric analysis revealed an almost sevenfold increase in a decade —from 250 records in 2016 to 1,673 in 2025—, concentrated in China and the United States, with Colombia absent from the leading producers despite the contamination already documented in its waters. The structure–property analysis showed that two descriptors, chain length and functional group, jointly govern human half-lives ranging from three days to more than fifteen years, the preferential binding to plasma proteins rather than lipids, and the mimicry of fatty-acid receptors and thyroid hormones; it further revealed that new-generation substitutes such as GenX retain and even worsen these properties, with degradation below 5% compared with 27% for PFOA under the same oxidative system. The technological assessment confirmed that conventional Colombian treatment trains do not remove PFAS (6.00 vs. 5.51 ng/L; p = 0.9138) and that granular activated carbon, anion exchange resins and solar photocatalysis are the most suitable routes for the national context. It is concluded that prioritizing a technology is premature while local occurrence data, the characterization of substitute compounds, and the local validation of destructive technologies remain pending, together with the regulatory development that would justify their implementation.
