Adherencia terapéutica en pacientes con enfermedades crónicas : una revisión crítica orientada a las causas de naturaleza administrativa

dc.description.abstractLa adherencia deficiente a los tratamientos a largo plazo compromete gravemente la efectividad del tratamiento, de manera que es un tema fundamental para la salud de la población desde la perspectiva de la calidad de vida y de la economía de la salud. Los factores asociados de manera positiva en la adherencia de naturaleza administrativa entre los que se destacan: Educación al paciente 34,6%; implementación del programa de Riesgo Cardiovascular 19.3%; relación médico Œ paciente 15,5%. Los factores de tipo administrativo son controlables desde la gerencia de las instituciones de salud y si se lograra un adecuado seguimiento y continuidad en el paciente frente a estos factores se lograría impacto favorable en la adherencia terapéutica frente a enfermedades crónicas como la Hipertensión Arterial y la Diabetes Mellitus que generan un gran costo económico y social en el sistema de salud. Por todo lo anterior se requiere que desde la gestión administrativa de cada una de las instituciones de servicios de salud haya la disposición para abordar estos factores que son propios de la administración y que generan un gran beneficio en comparación a los costos. Se debe invertir en educar al paciente sobre la enfermedad, en hacer seguimiento continuo de los pacientes con enfermedades crónicas por medio de programas de prevención secundaria de Riesgo cardiovascular y en escoger al personal idóneo con calidad y calidez en la atención de las personas que presentas estos diagnósticos.
dc.description.abstractenglishThe deficient adherence to the long-term treatments holds seriously the efficiency of the treatment, so that it is a fundamental topic for the health of the population from the perspective of the quality of life and of the health economics. The associate factors of a positive way in the adherence of administrative nature between those who are outlined: Education to the patient 34,6 %; implementation of the program of Cardiovascular Risk 19.3 %; relation medical - patient 15,5 %. The factors of administrative type are controllable from the management of the institutions of health and if a suitable follow-up was achieved and continuity in the patient opposite to these factors would achieve favorable impact in the therapeutic adherence opposite to chronic diseases as the Arterial Hypertension and the Diabetes Mellitus that generate a great economic and social cost in the system of health. By everything previous there is needed that from the administrative management of each one of the institutions of services of health there is the disposition to approach these factors that are own of the administration and that generate a great benefit in comparison to the costs. It is necessary to invest in educating the patient on the disease, in doing constant follow-up of the patients with chronic diseases by means of programs of secondary prevention of cardiovascular Risk and in choosing to the suitable personnel with quality and warmth in the attention of the persons that you present these diagnoses.
