Efectividad preventiva de la pregabalina en el síndrome doloroso crónico neuropático (sdcn) en mastectomía por cáncer

dc.description.abstractEn la población sometida a mastectomía por cáncer, la incidencia de síndrome doloroso crónico neuropático (SDCN) está entre 20 y 50%. Esta patología incrementa la morbilidad, retardando su rehabilitación y su retorno a las labores de la vida diaria con impacto en su calidad de vida. Dada la dificultad para el manejo de ésta entidad, es importante buscar mecanismos preventivos como lo es el uso de la pregabalina, un antineuropático del cual no se ha estudiado su efectividad preventiva en éste síndrome. Materiales y métodos: Estudio tipo Cohorte prospectiva, analítico incluyendo 74 pacientes cuya asignación no fue aleatoria sino por conveniencia. La presencia de SDCN se evalúo utilizando el cuestionario DN4 en 3 momentos: previo al alta, al mes y 90 días del posoperatorio, considerándose positiva una puntuación > a 4. Los datos fueron analizados con STATA. Resultados: La cohorte quedó conformada por 49% de participantes en el grupo expuesto (pregabalina), no hubo diferencias estadísticamente significativas respecto a las características basales entre los grupos. La incidencia de SDCN al egreso, 30 y 90 días postoperatorios fue del 3%, 13,51% y 13,51% en el grupo expuesto versus 5%, 20,51% y 15,79% en el grupo no expuesto, pero sin diferencias significativas entre ellos (p>0,05). Pregabalina mostró tendencia a ser efectiva y protectora con incidencias y Riesgo Relativo (RR) menores, sin diferencias significativas respecto al grupo no expuesto. Se presentaron efectos adversos en el 23% de los pacientes, el más frecuente fue somnolencia (17%) sin requerir la suspensión del medicamento. Conclusiones: La pregabalina administrada en forma profiláctica en pacientes que van a ser sometidas a mastectomía por cáncer tiene menores riesgos absolutos y relativos de presentar SDCN pero sin diferencias significativas. Se requiere realizar un estudio tipo ensayo clínico controlado que valide nuestros hallazgos.
dc.description.abstractenglishAmong the population undergoing mastectomy due to breast cancer, the incidence of chronic neuropathic pain syndrome (CNPS) is between 20 and 50%. This pathology increases the morbidity, delaying not only the rehabilitation process but also the return to daily activities with an impact in life quality. Given its difficult management, it is important to seek preventive measurements such as the use of pregabaline, an anti-neuropathic medication which preventive effectiveness in this syndrome has not been yet studied. Materials and Methods: Study type prospective cohort, analytic including 74 patients whose selection was not randomized but by convenience. The presence of CNPS was evaluated with the DN4 questioner during three moments: before discharge, at one month and 90 days postoperatory, considering positive a score > 4. The data was analyzed with STATA. Results: The cohort consisted of 49% of the subjects in the exposed group (pregabaline); there were no significant statistical differences among the basal characteristics of the groups. The incidence of CNPS at discharge, 30 and 90 days postoperatory was of 3%, 13,51% and 13,51% in the exposed group versus 5%, 20,51% and 15,79% in the unexposed group, but without significant differences between them (p>0,05). Pregabaline showed a tendency to be effective and protective with lower incidences and Relative Risk (RR), without significant differences compared to the unexposed group. Adverse effects were presented in 23% of the patients, the most common being somnolence (17%) without requiring discontinuation of the medication. Conclusions: Pregabaline administered prophylactically in patients that will be undergoing mastectomy due to breast cancer, has lower absolute risk and relative risk of presenting CNPS but without significant differences. A study type controlled clinical trial should be performed in order to validate our findings.
