Polimorfismo del gen de la enzima convertidora de angiotensina y riesgo de hipertensión arterial

dc.description.abstractOBJETIVO: Determinar la asociación entre el polimorfismo DD del gen de la ECA con la hipertensión arterial esencial. DISEÑO: Estudio Poblacional de casos y controles. LUGAR: Población de Bucaramanga. POBLACION: 486 pacientes del estudio CARMEN, los casos fueron los sujetos hipertensos, se seleccionó un numero igual de controles. INTERVENCION: Ninguna MEDICIONES: Polimorfismo DD del gen de la ECA como factor de riesgo para hipertensión arterial. RESULTADOS: 255 sujetos con hipertensión arterial, como controles solo 231 individuos por dificultades para obtener la muestra de la totalidad de los sujetos seleccionados. La edad promedio 52.9 años, el 32.5% fueron hombres. El promedio de TAS 133.7 y de TAD 78.8. Promedio de IMC 31.1. El 41,5% de la población realizaba actividad fisica regularmente, mayor en los controles 47 vs 36%. El antecedente familiar de hipertension en el 51.9%, mas frecuente en los casos 59 vs 43.8%. La prevalencia del polimorfismo DD, DI e II del gen de la ECA en los casos fue de 41.4%, 44.3% y 14.5%, en los controles fue 33.3%, 50.2% y 16.4% respectivamente. En el análisis de regresión logística el polimorfismo DD se asocio con HTA (OR 1.82 IC 95% 1.02-3.32 P 0.04). Otros factores asociados a HTA fueron: antecedente familiar de HTA (OR 1.78 IC 95% 1.21 Œ 2.62 P 0.03), sobrepeso (OR 1.45 IC 95% 1.02 Œ 2.34 P 0.03), tabaquismo (OR 3.82 1.39 Œ 10.43 P 0.03) y raza negra (OR 1.5 IC 95% 1.07 -2.21). La actividad física regular fue factor protector (OR 0.57 IC 95% 0.38 -0.84). CONCLUSION : El polimorfismo DD del gen de la ECA es un factor de riesgo para presentar hipertensión arterial, Otros factores de riesgo para HTA en esta población son: el sobrepeso, el tabaquismo y la raza negra. dc.description.abstractenglishOBJECTIVES: To determine the association between the DD polymorphism of the angiotensin converting enzyme and the arterial hypertension. DESIGN: It is case-control population study. SETTING: Bucaramanga´s population. PARTICIPANTS: 486 patients gather through the CARMEN study, the cases were all the patients with hypertension and an equal number of controls was randomly selected from the same population. INTERVENTION: none. MEASUREMENTS: DD polymorphism of the ACE as a risk factor of hypertension. RESULTS: 255 subjects with hypertension were included. Only 231 subjects were included as controls, due to difficulty in processing the DNA samples of some patients. The mean age was 52.9 years, 32.5% were men. The mean systolic blood pressure was 133.7 and the diastolic blood pressure was 78.8. The mean body mass index was 31.1. 41.5% of the population practiced some kind of regular physical activity, when analyzing case and controls separately the physical activity was higher in controls 47 Vs 36%. The family history of hypertension was present in 51.9% of the subjects, when analyzing cases and controls separately it was higher in the controls 59 Vs 43.8%. The prevalence of the DD, DI and II polymorphisms of the ACE in the cases were 41.4%, 44.3% and 14.5%, in the other side the prevalence of the polymorphisms in controls were 33.3%, 50.2% and 16.4%. In the logistic regression analysis, the DD polymorphism was associated with hypertension (OR 1.82 IC 95% 1.02-3.32 p 0.04). Other factors associated with hypertension were the family history (OR 1.78 IC 95% 1.21 Œ 2.62 p 0.03), overweight (OR 1.45 IC 95% 1.02-2.34 p =0.03), smoking (OR 3.82 IC 1.39-10.43 p 0.02) and black race (OR 1.5 IC 95% 1.07-2.21). physical activity was associated as a protective factor for hypertension (OR 0.57 IC 95% 0.38-0.84 p 0.006). CONCLUSION: in the present study DD polymorphism of the ACE gene is a risk factor for suffering hypertension, other risk factor were: overweight, smoking and black race. Physical activity was a protective factor for hypertension in this population
