Prevalencia de retraso de la cicatrización en relación al estado nutricional de pacientes hospitalizados por quemadura en el Hospital Universitario de Santander en el año 2019, Bucaramanga, Colombia

dc.description.abstract | Se realizó análisis prospectivo de historias clínicas de 169 pacientes quemados hospitalizados en HUS desde febrero 2019 a marzo 2020. Retraso en cicatrización analizado con factores de riesgo univariados y multivariados. Se determinaron variables de riesgo para retraso de cicatrización mediante regresión logística. El 54.76% fueron hombres, mediana de edad 38 (RIQ: 26-50.5). Prevalencia retraso de cicatrización 39.52%, mediana días de cicatrización: 17.5 (RIQ=12-26). La clasificación nutricional: desnutrición: 0.68%, sobrepeso: 39%, obesidad grado I: 13%, obesidad grado II: 3%, obesidad grado III: 2%, eutrófico: 64%. Variables asociadas a retraso de cicatrización fueron: quemadura eléctrica (OR: 6.77 P: 0.010 IC: 1.52 – 21.86), quemadura mayor al 25%SCT (OR: 13.65 P: 0.015 IC: 1.66 – 111.95), quemadura grado II profunda (OR: 2.25 P: 0.012 IC: 1.19 – 4.27), leucocitos >12000 (OR 2.72 P: 0.004 IC: 1.38 – 5.35), neutrófilos > 7500 (OR 1.98 P: 0.038 IC: 1.03 – 3.79) Linfocitos < 1000 (OR 8.78 P: 0.050 IC: 1.00227 - 77.01324), albúmina < 3.5gr/dl (OR 4.871795 P: 0.035 IC: 1.11961 - 21.19875), frecuencia cardiaca >90lpm (OR 3.23 P: 0.002 IC: 1.51 – 6.92), infección (OR 2.14 P: 0.043 IC: 1.02 – 4.50), injertos de piel (OR 12.41 P: <0.001 IC: 5.88 – 26.18) y no suplementación nutricional (OR 3.307 P: <0.001 IC: 1.71 – 6.38). | |
dc.description.abstractenglish | Prospective analysis of 169 patients clinic histories that were hospitalized with burn injuries in Santander´s College Hospital´s from february 2019 until march 2020. Cicatrization delay was analyzed with univariate and multivariate risk factors. Risk variables for cicatrization delay were determined by logistics regression. 54.76% were males, age average 38 years old (RIQ:26-50.5). Cicatrization delay prevalence was of 39.52%, days of cicatrization average: 17.5 (RIQ=12-26). Patients nutritional clasification: malnutrition: 0.68%, overweight: 39%, obesity: 39%, grade 1 obesity: 13%, grade 2 obesity: 3%, grade 3 obesity: 2%, eutrophic: 64%. Associated variables to cicatrization delay were: electric burn (OR: 6.77 P: 0.010 IC: 1.52 – 21.86), burn bigger than 25%SCT (OR: 13.65 P: 0.015 IC: 1.66 – 111.95), deep burn grade 2 (OR: 2.25 P: 0.012 IC: 1.19 – 4.27), number of leukocytes bigger than 12000 (OR 2.72 P: 0.004 IC: 1.38 – 5.35), neutrophils bigger than 7500 (OR 1.98 P: 0.038 IC: 1.03 – 3.79) lymphocites lesser than 1000 (OR 8.78 P: 0.050 IC: 1.00227 - 77.01324), albumin lesser than 3.5gr/dl (OR 4.871795 P: 0.035 IC: 1.11961 - 21.19875), heart rate >90lpm (OR 3.23 P: 0.002 IC: 1.51 – 6.92), infection (OR 2.14 P: 0.043 IC: 1.02 – 4.50), skin grafts (OR 12.41 P: <0.001 IC: 5.88 – 26.18) and not receiving nutritional supplement (OR 3.307 P: <0.001 IC: 1.71 – 6.38). The identification of cicatrization delay risk factors at the patient´s entry allows us to generate intervention estrategies from the general and nutritional point of view to obtain better results with this patients. | |
