dc.descriptionRESUMENLa sífilis congénita ocurre cuando la madre con sífilis transmite la infección al feto durante la gestación ya sea por víahematogena transplacentaria o durante el parto por contacto directo del neonato con lesiones genitales de la madre. EnColombia el plan nacional de salud del 2010 mostró sífilis gestacional que aumento de 1,3 por 1000 nacidos vivos en el2003 a un 5,4 en el 2008 y sífilis congénita de 0,9 por 1 000 nacidos vivos en 1998 a 2,6 en el 2008. Si la infección ocurreen una etapa temprana del embarazo, se pueden presentar: parto prematuro, retardo en el crecimiento intrauterinoy pérdidas fetales. La sífilis congénita se divide en temprana y tardia, dependiendo de la edad de aparición de lossíntomas, se define como temprana cuando se presentan en niños antes de los dos años de edad, las manifestacionesde la enfermedad pueden ser mas graves e incluyen: neunonitis, hepatomegalia, anemia, condiloma lata, petequias,osteocondritis, periostitis, meningitis aséptica, pseudoparálisis de Parrot, coriorretinitis, entre otras. Si la infecciónocurre en una etapa tardía después de los dos años, las manifestaciones de la enfermedad incluyen: queratitis intersticial,gomas en nariz y paladar, periostitis, articulación de Clutton, neurosífilis, dientes de Hutchinson, nariz en silla de montar,tibia en sable, entre otros. El diagnostico se realiza mediante pruebas no treponémicas inicialmente como la RPR o VDRLen la madre durante o al terminar la gestación, posteriormente se confirma con pruebas treponémicas como la FTA-ABSo TPHA. Las pruebas confirmatorias en el recién nacido son dos, el campo oscuro y los anticuerpos fluorescentes dirigidoscontra T. pallidum. El tratamiento es principalmente con penicilina durante el embarazo y en el niño con penicilinay eritromicina en caso de alergias. El objetivo principal del presente artículo es recordar a los médicos generales laimportancia de un control prenatal completo y del tratamiento oportuno de enfermedades infecciosas como la sífilis,para así prevenir complicaciones congénitas tan graves que puedan llegar a afectar la calidad de vida y el desarrollopsicomotor del neonato. Articulación de CluttonSUMMARYComprehensive approach to congenital syphilisCongenital syphilis occurs as a result of the infection of the fetus from an infected mother through gestation this maybe through placental hematologic spread or by direct contact to the mother’s genital lesions during labor. In Colombiathe national health plan for 2010 showed an increase of gestational syphilis (GS) in 1.3 by 1000 live births in 2003 and5.4 in 2008 an CS 0.9 by 1000 live births in 1998 to 2.6 in 2008. If infection occurs on an early stage of the pregnancy,could be present: preterm delivery, intrauterine growth retardation and fetal loss. The CS is divide into early and late,its depend of the age of onset of symptoms, defined as early when they occur in children before two year old, themanifestations of the disease could be more severe and include: neunonitis, hepatomegaly, anemia, condyloma lata,petechiae, osteochondritis, periostitis, aseptic meningitis, Parrot’s pseudoparalysis, chorioretinitis, among others. If theinfection occurs on a later stage, after the second year of age, the manifestations of the disease include: interstitialkeratitis, nose or palate gumma, periostitis, Clutton’s articulation, neurosyphilis, Hutchinson’s teeth, saddle nose, sabreshin, among others. The diagnosis is made through non-treponemal tests initially as RPR or VDRL in the mother duringor finishing the gestation, subsequently it is confirmed with treponemal tests (FTA-ABS or TPHA). Confirmed tests in thenew-born baby are two, the dark field and fluorescent antibodies against T. pallidum. The treatment is principally withpenicillin during the pregnant and in the children with penicillin and erythromycin in allergies cases. The main objectiveof this article is to remind general medical the importance of a complete antenatal control and a timely treatment ofinfectious disease like syphilis, in order to prevent congenital complications so severe that can affect quality of life andthe neonate psychomotor development.
