dc.description.abstract	El presente documento recopila la experiencia de práctica de Trabajo Social, realizada en la Fundación Esperanza Viva, en el municipio de Bucaramanga. El proceso de intervención se desarrolló con un grupo de jóvenes con patologías hematológicas y oncológicas residentes en los municipios del área metropolitana de Bucaramanga, cuyo principal objetivo fue orientar el proceso de formación de un grupo de apoyo social con el fin de promover la comunicación asertiva y el mejoramiento de las relaciones sociales de este grupo poblacional. La Teoría de Redes Sociales, aplicada en los grupos de apoyo se convierte en una herramienta para llevar a cabo el análisis de los vínculos que se generan entre las personas que conforman dichos grupos con determinadas características, siendo en este caso la condición de salud de los jóvenes y las repercusiones sociales que ha tenido la misma en ellos. Se realizó un acercamiento al marco internacional y nacional de la situación actual de la población que se intervino. Durante la experiencia de práctica académica en la Fundación Esperanza Viva, se llevó a cabo el proceso de conformación de un grupo social de apoyo, llegando a la primera etapa de formación grupal, además de se realizaron visitas domiciliarias como la base para elaborar el diagnóstico social de las personas que integraban el grupo. La evaluación de la experiencia permitió realizar la propuesta del modelo de atención social para la población juvenil con patologías hematológicas y oncológicas en el área metropolitana de Bucaramanga.
dc.description.abstractenglish	This document gathers the experience of social work practicum held at the Alive Hope Foundation in Bucaramanga. The interventive process was developed with a young group with hematologic and oncologic pathologies that live in the metropolitan área of Bucaramanga. The main objective was to orientate the process of formation of a group of social support in order to promote the assertive communication and the improvement of the social relations of this population group. The Theory of Social Networks, applied in the groups of support turns into a tool to carry out the analysis of the links that are generated between the persons who shape the above mentioned groups with certain characteristics, being in this case the condition of health of the young persons and the social repercussións that the same one has had in them. An approximation was realized to the international and national frame of the current situation of the population who was controlled. During the experience of academic practice in the Foundation Alive Hope, there was carried out the process of conformation of a social group of support, coming to the first stage of formation group, besides domiciliary visits were realized as the base to elaborate the social diagnosis of the persons who were integrating the group. The evaluation of the experience allowed to realize the offer of the model of social attention for the juvenile population with hematologic and oncologic pathologies in Bucaramanga's metropolitan área.
