| dc.description.abstract | Introducción: Las terapias con linfocitos CAR T anti-CD19 han transformado el abordaje de neoplasias hematológicas; no obstante, su eficacia a largo plazo se ve limitada por la diferenciación terminal y el agotamiento inducidos durante la expansión ex vivo. En este contexto, la modulación inmunometabólica se propone como una estrategia para optimizar la calidad funcional del producto celular. La metformina, modulador metabólico de uso clínico extendido, reprograma la bioenergética mitocondrial y favorece fenotipos de memoria en linfocitos T, lo que sustenta su evaluación traslacional en la manufactura de CAR T. Objetivo: Caracterizar en linfocitos CAR T anti-CD19 el inmunofenotipo, los cambios metabólicos y su capacidad citotóxica in vitro después del tratamiento con metformina. Metodología: Estudio experimental in vitro de linfocitos T purificados y CAR T anti-CD19 tratados con metformina o con UK-5099 (control positivo). La caracterización inmunofenotípica se realizó por citometría de flujo espectral y los datos se presentaron mediante UMAP. La cuantificación absoluta de genes implicados en rutas metabólicas y checkpoints se realizó por ddPCR. La evaluación funcional del perfil secretor (GM-CSF, TNF-α, IFN-γ, IL-2) y de la granzima B se determinó mediante tinción intracelular. Resultados: La metformina reconfiguró el inmunofenotipo hacia estados no terminales, indujo una firma compatible con beta oxidación y redujo progresivamente la expresión transcripcional de PD-1, TIM-3, LAG-3 y CTLA-4. Funcionalmente, se observó un incremento significativo de IL-2 y granzima B en CD4⁺ y CD8⁺ (p=0.0209 y p=0.0015–0.0039, respectivamente), junto con un aumento controlado de GM-CSF (p=0.0433–0.0455) y una tendencia a mantener niveles contenidos de IFN-γ (p=0.0304–0.0024). El control positivo UK-5099 mostró una reprogramación más potente con aumentos de granzima B (p=0.0015–0.0433) y GM-CSF (p=0.0015), validando la potencia de la intervención mitocondrial. Conclusiones: La modulación inmunometabólica mediante metformina integra metabolismo, inmunofenotipo y potencia efectora, posicionándose como una estrategia viable para optimizar la calidad funcional de productos CAR T con proyección traslacional en su manufactura. | |
| dc.description.abstractenglish | Introduction: Anti-CD19 CAR T lymphocyte therapies have transformed the management of hematologic neoplasms; however, their long-term efficacy is limited by terminal differentiation and exhaustion induced during ex vivo expansion. In this context, immunometabolic modulation is proposed as a strategy to optimize the functional quality of the cell product. Metformin, a widely used clinical metabolic modulator, reprograms mitochondrial bioenergetics and favors memory phenotypes in T lymphocytes, which supports its translational evaluation in CAR T manufacturing. Objective: To characterize, in anti-CD19 CAR T lymphocytes, the immunophenotype, metabolic changes, and in vitro cytotoxic capacity after treatment with metformin. Methodology: Experimental in vitro study of purified T lymphocytes and anti-CD19 CAR T treated with metformin or with UK-5099 (positive control). Immunophenotypic characterization was performed by spectral flow cytometry and data were presented using UMAP. Absolute quantification of genes involved in metabolic pathways and checkpoints was performed by ddPCR. Functional evaluation of the secretory profile (GM-CSF, TNF-α, IFN-γ, IL-2) and granzyme B was determined by intracellular staining. Results: Metformin reconfigured the immunophenotype toward non-terminal states, induced a signature compatible with beta-oxidation, and progressively reduced the transcriptional expression of PD-1, TIM-3, LAG-3 and CTLA-4. Functionally, a significant increase of IL-2 and granzyme B was observed in CD4⁺ and CD8⁺ (p = 0.0209 and p = 0.0015–0.0039, respectively), together with a controlled increase of GM-CSF (p = 0.0433–0.0455) and a tendency to maintain contained levels of IFN-γ (p = 0.0304–0.0024). The positive control UK-5099 showed a more potent reprogramming with increases of granzyme B (p = 0.0015–0.0433) and GM-CSF (p = 0.0015), validating the potency of the mitochondrial intervention. Conclusions: Immunometabolic modulation by metformin integrates metabolism, immunophenotype and effector potency, positioning itself as a viable strategy to optimize the functional quality of CAR T products with translational potential for their manufacture. | |
