Factores dietarios asociados a enfermedad cerebrovascular isquémica en adultos colombianos. Estudio multicéntrico de casos y controles

dc.description.abstract | La dieta ha sido propuesta como uno de los factores asociados a la Enfermedad Cerebrovascular (ECV), con un impacto variable entre las poblaciones. Objetivo: Evaluar la asociación entre factores dietarios y la ECV isquémica aguda en una muestra de pacientes adultos en Colombia. Metodología: Estudio de casos y controles incidentes, llevado a cabo en 357 controles y 348 casos hospitalizados con ECV isquémica en los servicios de urgencias de cinco ciudades. La ECV fue definida por la clínica y hallazgos imagenológicos. Se calculó odds ratios ajustados para la asociación entre el consumo de alimentos y ECV isquémica a través de tres modelos de regresión logística multivariados. Resultados: 12 alimentos fueron asociados con la ECV isquémica. Para el modelo en terciles se encontraron alimentos como: chocolate (p=0,015), gallina (p<0,000), pollo con piel (p=0,020), harina de trigo (p=0,025) y aguacate (p<0,000). Para el modelo dicotómico: atún aceite (OR 0,61 IC 95% 0,38-0,97), gallina (OR 2,39 IC 95% 1,52-3,78), pollo sin piel (OR 0,42 IC 95% 0,25-0,70), harina de trigo (OR 2,20 IC 95% 1,28-3,78) y arracacha (OR 1,90 IC 95% 1,19-3,03), Finalmente, para el modelo en veces/día: chocolate (OR 0,52 IC 95% 0,32-0,84), arroz (OR 1,84 IC 95% 1,17-2,90), atún aceite (OR 0,15 IC 95% 0,026-0,88), yuca (OR 2,45 IC 95% 1,36-4,40), gallina (OR 17,95 IC 95% 2,19-147,30), pollo con piel (OR 3,23 IC 95% 0,99-10,59), aguacate (OR 0,30 IC 95% 0,13-0,73) y manteca (OR 3,12 IC 95%1,18-8,24). Conclusiones: La dieta estaría asociada con la ECV isquémica en adultos colombianos. En el caso del chocolate, atún aceite, aguacate y pollo sin piel, su consumo podría constituir factores protectores, mientras la ingesta de harina de trigo, gallina, manteca, arroz, pollo con piel, arracacha y yuca podría aumentar el riesgo. | |
