dc.description.abstractObjetivo: Determinar la validez de los criterios de Rochester y los criterios cubanos del Hospital Juan M. Márquez (HJMM) para identificar los recién nacidos con alto riesgo para desarrollar una infección bacteriana severa (IBS). Materiales y métodos. Estudio de validación de pruebas diagnósticas prospectiva de neonatos con fiebre entre el primero de Febrero de 2006 y el 1 de enero de 2010. Se establecieron definiciones operativas para clasificar a los pacientes con IBS, luego se aplicaron las escalas de Rochester y de HJMM para determinar las características operativas de cada una. Resultados: Ingresaron 152 pacientes, noventa (59.2%) cumplieron criterios para IBS. Se encontró una sensibilidad 87.5% (IC 95% 77.95%-97.05%), especificidad 27.87% (IC 95% 15.80%- 39.94%), Valor Predictivo Positivo(VPP): 52,69% (IC 42%-63.37%), Valor Predictivo Negativo(VPN): 70.83% (IC 95% 0.010 a 0.289) para los criterios de Rochester y un mejor rendimiento con los criterios cubanos del HJMM con sensibilidad: 92.86% (IC 95% 85.22%-100%), especificidad: 36.07% (IC 95% 23.20%-48.94%), VPP: 57.14% (IC 95% 46.43%-63.96%) y VPN: 84.62% (IC 95% 68.82-100%) Conclusión: No existe hasta el momento la escala predictiva ideal para el diagnóstico de infección bacteriana severa en el neonato febril. Esto apoya la necesidad de continuar con el abordaje diagnóstico completo de sepsis y el manejo con antibioticoterapia intrahospitalaria en espera de reporte de hemocultivos con observación estrecha del estado clínico del recién nacido
dc.description.abstractenglishAim: To determine the validity of the Rochester criteria and the Cuban criteria of the Juan M. Marquez Hospital (JMMH) in identifying newborns at high risk of developing a severe bacterial infection (SBI). Methods: Validation of diagnostic tests study of febrile newborns between February of 2006 and January of 2010. Patients were classified into presence or absence of severe bacterial infection (SBI) according to operative definitions and the both Rochester and JMMH escales were applied to evaluate its operative charasteristics. Results: 152 patients were selected; 40 (59.2%) met the SBI criteria. We found a sensitivity of 87.5 (CI 95% 77.95% - 97.05%), specificity of 27.87%(CI 95% 15.80-39.94%). Positive Predictive Value (PPV) of 52.69% (CI 42%-63.37%), Negative Predictive Value (NPV) of 70.93% (CI 95% 0.010 to 0.289) for the Rochester criteria and a better performance of the Cuban criteria of the JMMH with a sensitivity of 92.86% (CI 95% 85.22% - 100%) specificity of 36.07% (CI 95% 23.20%-28.94%), PPV: 57.14% (CI 95% 46.43%-63.96%) and NPV of 84.62% (CI 95% 68.82-100%). Conclusion: There is no evidence of an ideal scale for the diagnostic of SBI in the febrile newborn. This findings support the need of keeping a complete diagnosis undertake of the infection and the intra hospitality antibiotic therapy management while waiting for the hemocultive reports with a close observation of the clinical state of the newborn.
