Reproducibilidad de las pruebas serológicas para el diagnóstico de la infección por Chagas en gestantes de zona endémica en Santander

dc.description.abstract | El diagnóstico de infección por Trypanosoma cruzi (T. cruzi) en fase crónica, está dado por pruebas serológicas descritas en la literatura en cuanto a la validez entendida ésta como sensibilidad y especificidad. No obstante la confiabilidad de las mismas, determinada por la reproducibilidad de éstas herramientas diagnósticas, no ha sido documentada en estudios realizados de la mejor manera , es decir, con muestreos adecuados. Objetivo: Evaluar la reproducibilidad de las pruebas serológicas (ELISA, IFI y HAI) para el diagnóstico de la infección por chagas en gestantes de zona endémica en Santander. Metodología: Se realizó un estudio de evaluación de tecnología diagnóstica, en el componente de reproducibilidad y acuerdo, en gestantes de zona endémica de Santander para la enfermedad de Chagas, quienes fueron seleccionadas mediante muestreo de corte transversal, para determinar reproducibilidad de las pruebas ELISA, IFI y HAI. Se analizaron las variables sociodemográficas para caracterizar la muestra. Se usó el software Stata versión 10.0 para análisis estadísticos. Resultados: Se evaluaron 777 muestras. En muestra de suero ELISA ( punto de corte 0,3), IFI (punto de corte 1/32) y HAI (punto de corte 1/16) presentaron kappa mayor a 0,8; no existen diferencias estadísticamente significativas entre las tres pruebas evaluadas. En eluído sanguíneo obtenido de papel filtro, el kappa estuvo por debajo de 0,8. Conclusiones: las tres pruebas serológicas en muestra de suero presentan una reproducibilidad perfecta, determinada mediante kappa, por lo que la selección de cualquiera de ellas es útil para establecer el diagnóstico de infección por T. cruzi. dc.description.abstractenglish | Diagnosis of infection with Trypanosoma cruzi (T. cruzi) in chronic phase, is given by serological tests that have been widely described in the literature as to its validity understood these characteristics as sensitivity and specificity. However the reliability thereof, determined by the reproducibility of these diagnostic tools has not been documented in studies performed adequately, transverse sampling. Objective: To evaluate the reproducibility of serological tests (ELISA, IFI and HAI) for the diagnosis with Chagas infection in pregnant women in endemic área in Santander. Methods: We performed a study evaluating diagnostic technology for reproducibility and agreement, in pregnant women living in endemic área of Santander for Chagas disease, who were selected by transverse sampling random, to determine the reproducibility of the ELISA, IFA and HAI. Sociodemographic variables were analyzed to characterize the sample. Analyzes were performed in the Stata software versión 10.0. Results: Seven hundred and seventy seven samples were evaluated. ELISA serum sample ( cutoff of 0.3), IFI (cutoff at dilution 1/32) and HAI (with cutoff at 1/16) had kappa values greater to 0 , 8 and no statistically significant differences between the three tests evaluated. kappa in eluate was below to 0.8. ELISA is recommended for diagnosis Chagas disease. Conclusións: the three serological tests evaluated serum sample present perfect reproducibility, determined by weighted kappa, so selecting any of them is useful for diagnosis of infection with T. cruzi. Due to Elisa its simplicity and cost is recommended as test of choice for screening programs. | |
