dc.description.abstractLa violencia sexual contra las mujeres es un problema de salud pública. OBJETIVO: Caracterizar las variables demográficas y clínicas de las mujeres y niñas atendidas por presuntos delitos sexuales en la unidad básica de Bucaramanga del Instituto Nacional de Medicina Legal y Ciencias Forenses (INMLCF), durante el 2014. MÉTODOS: Se consolidaron variables demográficas, del evento, clínicas, paraclínicos y el abordaje terapéutico registradas durante el dictamen pericial de los casos por presuntos delitos sexuales en 553 niñas y mujeres. RESULTADOS: 43 casos/100.000 habitantes fue la tasa para el área Metropolitana de Bucaramanga. 10 a 14 años fue el grupo de edad más afectado, 80% de las >14 años estaban solteras. 8,3% consultaron el mismo día de los hechos. 85,7% reportó agresión sexual en varias ocasiones. 65,5% ocurrieron en la vivienda de la víctima o victimario. 94,9% de los agresores fueron hombres y 87% eran familiares (padrastros/padres/tíos). En 31,5% hubo penetración y en 79% otras actividades sexuales (más frecuente tocamiento del área genital). 14% presentaron síntomas (disuria y dolor genital) y en 14,5% se encontraron laceraciones y equimosis en extremidades, 6% presentaron alguna lesión genital (más frecuente horquilla vulvar y vagina); 22% tenían himen íntegro elástico y hubo desgarro reciente en 1,2%. 2,3% presentó embarazo presuntamente secundario. Respecto al abordaje, 21% consultó previamente a una Institución de Salud y después del peritaje fueron remitidas para valoración por Psiquiatría o Psicología forense el 28,7% y 15%, respectivamente. La solicitud y el registro de paraclínicos para la identificación de enfermedades de transmisión sexual fue baja (<15%). CONCLUSIÓN: Las características sociodemográficas fueron similares a las reportadas en Colombia. Es el primer trabajo que describe los hallazgos físicos que se presentan en delitos sexuales en mujeres colombianas. Son pocos los hallazgos físicos porque el tocamiento es la principal actividad sexual utilizada como agresión.

METHODS: Demographic, event, clinical and paraclinical variables and the therapeutic approach recorded during the expert opinion of the cases for alleged sexual crimes were consolidated in 553 girls and women. RESULTS: 10 to 14 years old was the most affected age group, 80% of the >14 years were single. 8.3% consulted on the same day of the events. 85.7% reported sexual assault on several occasions. 65.5% occurred in the dwelling of the victim or victimizer. 94.9% of the aggressors were men and 87% were family members (stepparents / parents / uncles). In 31.5% there was penetration and in 79% other sexual activities (more frequent, touching of the genital area). 14% presented symptoms (dysuria and genital pain), and in 14.5%, lacerations and ecchymosis were found in extremities, 6% had some genital lesions (more frequent vulvar fourchette and vagina); 22% had elastic integral hymen and there was a recent tear in 1.2%. 2.3% presented presumed secondary pregnancy. Regarding the approach, 21% previously consulted with a Health Institution and after the evaluation were referred for evaluation by Psychiatry or Forensic Psychology 28.7% and 15%, respectively. The application and registration of paraclinics for the identification of sexually transmitted diseases was low (<15%). CONCLUSION: The sociodemographic characteristics were similar to those reported in Colombia. This is the first that describes the physical findings that occur in sexual crimes in colombian women. There are few physical findings because touching is the main sexual activity used as an aggression.
