Prevalencia de hipotiroidismo primario en la población mayor de 14 años del municipio de Santa Fe de Antioquia, departamento de Antioquia en el año 2006

dc.description.abstract | ANTECEDENTES: No existe suficiente información sobre la prevalencia de enfermedades tiroideas, en especial de hipotiroidismo, en la población Colombiana. OBJETIVO: Determinar la prevalencia de hipotiroidismo en la población mayor de 14 años de Santa Fe de Antioquia y la relación entre: (1) hipotiroidismo y niveles de lípidos e (2) hipotiroidismo y sintomatología. DISEÑO: Estudio analítico de corte transversal. METODOLOGIA: Se realizó un muestreo probabilístico aleatorio simple, tomando como población las personas mayores de 14 años que aparecen registradas en el sistema de selección de beneficiario (SISBEN). A las personas seleccionadas se aplicó el instrumento de recolección de información diseñado y fueron tomadas muestras sanguíneas para las mediciones. La recolección de la muestra fue realizada entre enero y mayo de 2006. RESULTADOS: Se encuestaron 260 individuos de los cuales 245 individuos cumplieron con los criterios de inclusión y exclusión. Se encontró una prevalencia de hipotiroidismo de 10,61% (IC 95% = 6,76 – 14,47), la mayoría de los cuales correspondieron a hipotiroideos subclínicos (6,12%; IC 95% = 3,12 – 9,12). En el análisis bivariado, la presencia de parestesias y el déficit de memoria como síntomas y el aumento de tamaño tiroideo, características toscas faciales, palidez, macroglosia, bradicinesia, piel seca e hiporreflexia como signos, fueron más frecuentes en individuos con TSH elevada. El promedio de colesterol total, colesterol – LDL y triglicéridos fue significativamente más alto entre los sujetos hipotiroideos. El nivel de colesterol – HDL no varió significativamente. En el análisis multivariado, teniendo en cuenta variables clínicas y de laboratorio se encontró asociación entre los niveles elevados de colesterol total (> 200 mg/dL), el colesterol - HDL bajo (< 40 mg/dL), la hiporreflexia, el aumento de tamaño tiroideo, la piel seca y muy probablemente el déficit de memoria con el hipotiroidismo. CONCLUSIONES: La prevalencia de hipotiroidismo en la población de Santa Fe de Antioquia de 10,6% (IC 95% = 6,76 – 14,47), esta particularmente elevada con respecto a otras prevelencias reportadas en la región. Este estudio sugiere que ciertas manifestaciones clínicas y pruebas de laboratorio pueden sugerirle al clínico la presencia de estados de hipofunción tiroidea. | |
dc.description.abstractenglish | BACKGROUND: There is no sufficient information about the prevalence of thyroidal diseases, especially of hypothyroidism, in the Colombian population. OBJECTIVE: To determine the prevalence of hypothyroidism in the population older than 14 years of Santa Fe of Antioquia and the relationship among: (1) hypothyroidism and lipids levels and (2) hypothyroidism and symptoms. DESIGN: Cross – sectional study. METHODOLOGY: The participants were selected for probabilistic sampling simple random, taking as population the subjects older than 14 year of age that appear registered in the system of beneficiary's selection (SISBEN). Participants complete the instrument of recollection of information and sanguine samples were taken for the measures. Between January and May of 2006 the recollection of the sample were completed. RESULTS: Two hundred forty five subjects complete for inclusion and exclusion criteria. The prevalence of hypothyroidism in this population was 10,61% (IC 95% = 6,76–14,47), most of which corresponded to subclinicals hypothyroids (6,12%; IC 95% = 3,12–9,12). In the analysis bivariate, the paraesthesia presence and the poorer memory as symptoms and the increase of thyroid size, facial rough characteristic, paleness, high tongue, slow movements, dry skin and reflex delay like signs, they were more frequent in individuals with high TSH. The average of total cholesterol, cholesterol–LDL and triglycerides were significantly higher among the subject hypothyroids. The level of cholesterol–HDL didn't vary significantly. In the analysis multivariate, including clinical and paraclinical variables show association among the high levels of total cholesterol (> 200 mg/dL), the cholesterol - HDL low (< 40 mg/dL), the reflex delay, the increase of thyroid size, the dry skin and very probably the poor memory with the hypothyroidism. CONCLUSIONS: The prevalence of hypothyroidism in the population of Santa Fe of Antioquia of 10,6% (IC 95% = 6,76–14,47), this particularly high with regard to other prevalence reported in the region. This study suggests that certain clinical manifestations and laboratory tests can suggest to the clinical the presence of states of thyroid hypofunction. | |
