Análisis biomecánico en fracturas claviculares del tercio medio tratadas con placa osteosíntesis en la posición superior y anteroinferior

dc.description.abstract | El método quirúrgico para reparar lesiones claviculares desplazadas del tercio medio utilizando placas de osteosíntesis, representa un motivo de polémica en términos de la superficie (superior o anteroinferior) clavicular en la que se debe fijar el implante para que ofrezca mayor estabilidad; por tal razón, se permite dar respuesta para aclarar dicha polémica, a partir del estudio de la biomecánica del cuerpo humano; lo cual, puede significar un buen resultado en la atención médica y calidad de vida del paciente que presenta dicha condición. Diez muestras claviculares humanas, fueron manipuladas y expuestas a un ensayo mecánico de flexión para obtener la carga máxima de falla, y una lesión desplazada a nivel del tercio medio. Una vez concluida esta fase y mediante criterios de selección estipulados, se tomó un espécimen para obtener mediante una tomografía computarizada los datos DICOM (Digital Imaging and Communication in Medicine) que fueron necesarios para reconstruir el hueso tridimensionalmente. Se modelarón placas LCP (Locking Compression Plate) con el fin de ser fijadas en las posiciones superior y anteroinferior del modelo clavicular. Luego con un análisis de elementos finitos, se observaron los esfuerzos y modos de deformación máximos implicados en cada una de las fijaciones tras someter a cargas fisiológicas el material óseo; finalmente se procedió a encontrar diferencias significativas en las dos posiciones donde se fijó el material de osteosíntesis para dar respuesta al objetivo de esta investigación. | |
dc.description.abstractenglish | The surgical method to repair displaced clavicular lesions of the middle third using osteosynthesis plates represents a controversial motif in terms of the clavicular (superior or anteroinferior) surface in which the implant must be fixed in order to offer greater stability; for this reason, it is allowed to give an answer to clarify this controversy, to from the study of the biomechanics of the human body; which can mean a good result in the medical care and quality of life of the patient who presents this condition. Ten human clavicular samples were manipulated and exposed to a mechanical bending test to obtain the maximum failure load, and a displaced lesion at the level of the middle third. Once this phase was concluded and by means of stipulated selection criteria, a specimen was taken for so that by means of a computerized tomography to obtain the DICOM (Digital Imaging and Communication in Medicine) data that was necessary to reconstruct the bone three-dimensionally. LCP (Locking Compression Plate) plates were modeled to be fixed in the superior and anteroinferior positions of the clavicular model. Then with a finite element analysis, the maximum stresses and deformation modes involved in each of the fixations were observed after subjecting the bone material to physiological loads; finally, we proceed to find significant differences in the two positions where the osteosynthesis material was fixed to give an answer to the objective of this investigation. | |
