dc.description.abstract | Los recién nacidos prematuros tardíos representan una población que ha sido subestimada en la práctica clínica, sin embargo, la evidencia ha demostrado que tienen un mayor riesgo de morbilidad respiratoria en comparación con recién nacidos a término, un hecho que ha planteado la controversia de igualar estas dos poblaciones en la selección del método de soporte ventilatorio en caso de dificultad respiratoria. Actualmente, ningún estudio en la literatura ha proporcionado evidencia sobre el método ventilatorio óptimo para el síndrome de dificultad respiratoria (SDR) en la población de prematuros tardíos. El objetivo de este estudio es comparar los resultados de recién nacidos prematuros tardíos con SDR de acuerdo con el método de soporte ventilatorio recibido. Se realizó un estudio de cohorte retrospectivo en la unidad de cuidados neonatales del Hospital Universitario de Santander y la Clínica Materno-Infantil San Luis. Se incluyeron todos los recién nacidos con edades gestacionales entre 33-36 6/7 semanas que nacieron en el período 2013-2016 y desarrollaron un RDS temprano. Se realizó un análisis de regresión logística multinomial. En total se incluyeron 382 pacientes. Se identificaron siete métodos de soporte ventilatorio como la opción de inicio para el manejo del SDR, entre ellos, solo el tratamiento con INSURE mostró un menor riesgo de fracaso estadísticamente significativo. En Conclusión el uso de INSURE como terapia de primera línea para SDR en recién nacidos prematuros tardíos mostró un beneficio significativo sobre otros métodos ventilatorios incluso cuando se ajustaron mediante variables de confusión, con tasas similares de complicaciones. INSURE debe tenerse en cuenta para el tratamiento del SDR temprano en esta población. | |
dc.description.abstractenglish | Late preterm infants represent a population that has been underestimated in clinical practice, however, evidence has shown that has a higher risk of respiratory morbidity compared to term newborns , a fact that has raised the controversy of equaling these two populations in the selection of ventilatory support method in case of respiratory distress. Currently, no study in the literature has provided evidence on the optimal ventilatoty method for respiratory distress syndrome (SDS) in the population of late preterm infants. The aim of this study is to compare the outcomes of late preterm infants with RDS according to the method of ventilatory support they received. A retrospective study was conducted in the neonatal care unit of the University Hospital of Santander and the Clinica Materno Infantil San Luis. All newborns with gestational ages between 33-36 6/7 weeks who were born in the 2014-2016 period that develop early RDS were included. A multinomial logistic regression analysis was performed. In total, 382 patients were included. Seven methods of ventilatory support were identified as the starting option for managing SDR, among them, treatment with INSURE therapy shows a significant statistically lower risk of failure. In conclusion the INSURE therapy as the first line therapy for SDR in late preterm infants must be taken into account for the treatment of early SDR in this population. | |
