dc.description.abstractEl síndrome febril sin foco constituye una causa de preocupación frecuente para padres y cuidadores, siendo el motivo de consulta en 1 de cada 5 atenciones en los servicios de urgencias pediátricas. El objetivo del proyecto es evaluar el rendimiento de cuatro escalas predictoras para bajo riesgo de infección bacteriana grave (IBG) en los menores de 90 días con síndrome febril agudo sin foco que consultan al servicio de urgencias pediátricas del Hospital Universitario de Santander (HUS) y la Clínica San Luis (CSL). Las escalas elegidas en orden de publicación fueron los criterios de Rochester, la escala clínica de Mendoza, el Step by Step modificado (sin procalcitonina) y el IBI score; porque son comparables, con sensibilidad (S) entre 81 - 92% y valor predictivo negativo (VPN) entre 95 - 99%. Se incluyeron un total de 171 pacientes, 45 del HUS y 126 de la CSL, durante agosto/2022 a septiembre/2023; el promedio de edad fue 9 días, recién nacidos (1-28 días) en el 71,3 % y lactantes (29 a 89 días) en el 28,7 %. la prevalencia de IBG fue del 35,7 %, la más frecuente fue la infección de vías urinarias en el 17,5 %, en menor proporción la bacteriemia en el 2,3 % y meningitis en el 1,2 %. se tomaron muestras para cultivo en el 80,7 %, con aislamiento microbiológico en el 31,8 % de estas, siendo la Escherichia Coli el patógeno predominante en el 54,5 %. El rendimiento diagnóstico de las escalas evaluadas en nuestra población es inferior al reportado en la literatura, al compararlas son los criterios de Rochester los que tiene el mejor desempeño con S de 72,3 %, VPN del 76,9% y LR - en 0,49, y este comportamiento se mantiene al analizarlas por subgrupos de edad: neonatos (VPN: 76,9 % - LR-: 0,61) y lactantes (VPN: 76,9 % - LR-: 0,29). A su vez en el grupo de lactantes la escala Step by Step modificada también muestra validez con VPN del 70,4 % y un LR- en 0,40.
dc.description.abstractenglishThe febrile syndrome without a source represents a frequent cause of concern for parents and caregivers, accounting for 1 in 5 visits to pediatric emergency services. The aim of the project is to assess the performance of four predictive scales for low risk of severe bacterial infection (SBI) in children under 90 days with acute febrile syndrome without a source, treated at the pediatric emergency services of the Hospital Universitario de Santander (HUS) and the Clínica San Luis (CSL). The chosen scales, in order of publication, were the Rochester criteria, the Mendoza clinical scale, the modified Step by Step (without procalcitonin), and the IBI score; chosen for their comparability, with sensitivity (S) between 81 – 92 % and negative predictive value (NPV) between 95 – 99 %. A total of 171 patients were included, 45 from the HUS and 126 from the CSL, between August/2022 to September/2023; average age was 9 days, newborns (1-28 days) in 71.3% and infants (29 to 89 days) in 28.7 %. The prevalence of SBI was 35.7 %, the most frequent was urinary tract infection in 17.5 %, followed by bacteremia at 2.3 %, and meningitis at 1.2 %. Cultures were taken in 80.7 % of cases, with a microbiological isolation in 31.8 % of them, and Escherichia coli being the predominant pathogen at 54.5 %. The diagnostic performance of the scales evaluated in our population is lower than reported in the literature. When compared, the Rochester criteria performed the best with a S of 72.3 %, a NPV of 76.9 %, and a likelihood ratio (LR-) of 0.49. This trend persists when analyzing them by age subgroups: neonates (NPV: 76.9 %, LR-: 0.61) and infants (NPV: 76.9 %, LR-: 0.29). In the infant group, the modified Step by Step scale also shows validity with a NPV of 70.4 % and an LR- of 0.40.
