Angioplastia/ stent para el manejo de la estenosis carotidea : reporte de casos en la Fundación Cardiovascular de Colombia : 2005-2007

dc.description.abstractLa estenosis de la arteria carótida interna es la principal causa de accidente cerebrovascular isquémico. El manejo de la estenosis de arteria carótida interna con angioplastia / stent es una nueva alternativa de tratamiento para los pacientes con esta patología con comorbilidades importantes que hacen de la endarterectomía carotídea un procedimiento de alto riesgo. El siguiente estudio describe el comportamiento clínico de los pacientes con estenosis de la arteria carótida interna, sintomáticos o asintomáticos, que han sido manejados con angioplastia / stent en la Fundación Cardiovascular de Colombia en el periodo de noviembre de 2005 a mayo de 2007. Se encontraron un total de 75 pacientes sometidos a cirugía por estenosis carotídea, de los cuales 24 se les practicó angioplastia / stent de la carótida interna. El 75% (18) de los pacientes fueron de sexo masculino, el promedio de edad fue de 68.4 ± 8.07 años (rango: 54 - 81 años), el 78,33% (14) de las angioplastias y stent se practicaron del lado derecho y el 41,67% (10) restantes del lado izquierdo. El promedio de estenosis carotídea encontrado fue del 75,2 ± 14,1% (Rango: 30 - 90 %). El 54% de los pacientes eran sintomáticos y el 46% asintomáticos. No hubo complicaciones perioperatorias ni en los primeros 30 días. Se presentó una mortalidad a los 288 días extrainstitucional del grupo de los asintomáticos y dos complicaciones correspondientes a disartria a los 347 días y de amaurosis a los 307 días en el grupo de los sintomáticos. Para la Fundación Cardiovascular de Colombia, la angioplastia / stent de la arteria carótida interna para el manejo de la estenosis carotídea tanto sintomática como asintomática es un procedimiento seguro en los pacientes de alto riesgo dado por sus comorbilidades.
dc.description.abstractenglishThe stenosis of the internal carotid artery is the main cause of ischemic stroke. The operation of the internal carotid artery stenosis with angioplasty / stent is a new treatment alternative for patients with this disorder with significant comorbidities that make a carotid endarterectomy in high-risk procedure. The next study describes the clinical behavior of patients with stenosis of the internal carotid artery, symptomatic or asymptomatic, which have been managed with angioplasty / stent in the Cardiovascular Foundation of Colombia in the period November 2005 to May 2007. We found a total of 75 patients undergoing carotid surgery for stenosis, of which 24 were practiced angioplasty / stent in the internal carotid. 75% (18) of the patients were male, the average age was 68.4 ± 8.07 years (range: 54 - 81 years) the 78.33% (14) of angioplasty and stenting were performed on the side right and 41.67% (10) remaining on the left side. The average carotid stenosis found was 75.2 ± 14.1% (range: 30 - 90%). 54% of patients were symptomatic and asymptomatic for 46%. There were no perioperative complications in the first 30 days. A presentation was mortality at 288 days of extrainstitutional of asymptomatic and two complications related to dysarthria to 347 days and 307 days to amaurosis in the symptomatic group. For Foundation Cardiovascular de Colombia, angioplasty / stent of the internal carotid artery for the management of both symptomatic carotid stenosis as asymptomatic is a safe procedure for patients at high risk because of their comorbidities.
