dc.description.abstract | En las últimas dos décadas ha aumentado la mortalidad cardiovascular por Infarto Agudo del Miocardio (IAM). Se han desarrollado indicadores para establecer riesgo posinfarto, tales como la Variabilidad de la Frecuencia Cardiaca (VFC), pero no hay claridad sobre su utilidad. La VFC reducida después del IAM indica dismunición en la actividad vagal, aumento del tono simpático e inestabilidad eléctrica, predisponiendo a arritmias cardìacas y deterioro cardiovascular. OBJETIVO: Evaluar el valor pronóstico de la VFC sobre la morbimortalidad durante la fase aguda del IAM. METODOLOGÌA: Es un estudio de cohortes concurrente, correspondiente a un análisis interino preestablecido del 50% del tamaño de la muestra (87 pacientes). Se determinò un nivel de confianza del 95%, un poder del 80% y un riesgo relativo de 2.0 para los desenlaces. Se tomaron las señales electrocadiogràficas de los pacientes en las 12 horas de ingreso a urgencias; por medio de un software se determinò la VFC. Se realizò el seguimiento durante la fase hospitalaria, determinando los desenlaces de morbimortalidad . RESULTADOS Y CONCLUSIONES: La fracciòn de eyecciòn baja mostrò riesgo para desarrollar complicaciones. La VFC baja en el grupo de pacientes complicados con un primer IAM demostrò tener valor pronostico de morbimortalidad . Se continùa el estudio hasta completar el tamaño de muestra. El uso de nuevas herramientas para análizar señales biológicas , amplìa la capacidad predictiva y orienta la toma conductas mèdicas tempranas tendientes a disminuir la morbimortalidad en estos pacientes. | |
dc.description.abstractenglish | In the last two decades the cardiovascular mortality has increased by acute myocardial infarction (AMI). The indicators have been developed to establish risk after AMI, such as heart rate variability (HRV), but there is not clarity about its utility. The low HRV after AMI indicates reduction in the vagal activity , sympathetic tone increased and electric inestability, predisposing to heart arrhythms and cardiovascular impairment. OBJECTIVE: To evaluate the prognostic value of the HRV on the morbimortality during the acute phase of AMI. METHODS: It is a concurrent cohorts study corresponding to a interim analysis with 50% of the size of the sample (87 patients). We estimate a confidence interval of 95%, an statistical power of 80% and a relative risk of 2.0 for the outcomes. We took the electrocardiographic signals of the patients into the 12 hours of entry to urgencies; we determined the 13 HRV using a software . The following was done during the hospitalary phase, determining the morbomortality outcomes. RESULTS AND CONCLUSIONS: The low ejection fraction showed risk increased to develop complications. The low HRV in the complicated patients with a first AMI showed high prognostic value to predice morbimortality. The study is continued up to complete the sample size. The use of new tools to analyze biological signals increase the predictive capacity and it guides to take early medical decisions to diminish the morbomortality in these patients. | |
