Supervivencia a 5 anos de las mujeres con cáncer de mama de Bucaramanga y su área metropolitana según el estadio clínico

dc.description.abstract | El cáncer de mama es un problema de salud pública en el Área Metropolitana de Bucaramanga (AMB), desconocemos la distribución del estadio clínico, la supervivencia y sus determinantes. Los objetivos fueron determinar las tasas global y específicas de la supervivencia a 5 años y establecer la asociación con el estadio clínico. Para ello se estudió una cohorte cerrada retrospectiva con 719 casos incidentes del Registro Poblacional de Cáncer del Área Metropolitana de Bucaramanga (RPC-AMB) entre 2001 y 2004, se estimó la supervivencia a cinco años y su asociación con el estadio clínico al momento del diagnóstico (TNM de la American Joint Committee on Cáncer (AJCC)) utilizando modelo de regresión de Cox, ajustando por factores pronóstico y estratificando por tipo de régimen al sistema de seguridad social (SSS). La tasa de letalidad fue de 7,0/1000 personas mes de seguimiento (IC95%: 6,1-7,9). La tasa global de supervivencia a 5 años estimada fue 66,8%, siendo en estadios tempranos de 84,2%, para los avanzados de 64,8% y para el metastásico de 17,6%. En el modelo de Cox ajustado por variables clínicas, histopatológicas y terapéuticas y estratificando por SSS, se encontró una asociación significativa con el estadio clínico (HR avanzados vs. Tempranos =1,93) IC 95% (1,24- 3,01), (HR metastásico vs. Temprano = 9,05) IC95% (5,65-14,49) y con el tiempo al inicio de tratamiento (HR 30 días versus 61-90 días= 0,59) IC95%(0,40-0,87) La supervivencia del cáncer de mama en AMB se asocia independientemente con estadio clínico al momento del diagnóstico y el tiempo para el inicio de las modalidades terapéuticas. | |
dc.description.abstractenglish | Breast cancer is a public health problem in the metropolitan area of know the distribution of clinical stage, survival and its determinants. The objectives were to determine rates of overall and specific survival at 5 years and establish association with clinical stage. To do this we studied a retrospective cohort with 719 cases closed incidents Cancer Registry of the Metropolitan Area of Bucaramanga (RPC-AMB) between 2001 and 2004, an estimated five-year survival and its association with clinical stage at diagnosis (TNM American Joint Committee on Cancer (AJCC)) using Cox regression model, adjusting for prognostic factors and stratified by type of the social security system (SSS). The fatality rate was 7,0 / 1,000 person-month follow up (95% CI 6,1 to 7,9). The overall 5-year survival estimated was 66,8%, still in early stages of 84,2% for advanced 64,8% and for metastatic 17,6%. In the Cox model adjusted for clinical variables, histopathological and therapeutic stratification by SSS, we found a significant association with clinical stage (advanced vs HR. Early = 1,93) (95% CI 1,24 to 3,01) (metastatic vs HR. Early = 9,05) 95% (from 5,65 to 14,49) and the time at the start of treatment (HR 61-90 days versus 30 days = 0,59) 95% (0,40-0,87). The survival of breast cancer in AMB was independently associated with clinical stage at diagnosis and time to the start of therapeutic modalities. | |
