Diseño y validación de un instrumento para medir estigma en personas con tuberculosis del área metropolitana de Bucaramanga

dc.description.abstractLa tuberculosis continúa siendo un problema en salud pública con alta carga de la enfermedad, principalmente en países de medianos y bajos ingresos. Existen factores que limitan su adecuado control como el diagnóstico inoportuno y la no adherencia al tratamiento. La magnitud del efecto del estigma en el control de la tuberculosis es desconocida o quizás subestimada porque no hay instrumentos válidos para cuantificar y medir este fenómeno, lo que dificulta la exploración de sus efectos en escenarios endémicos de la enfermedad. Objetivo: Diseñar y validar una escala para medir estigma hacia la tuberculosis en personas que residen en el área metropolitana de Bucaramanga. Metodología: Se realizó un estudio de evaluación de tecnología diagnóstica, con diseño y validación de un instrumento. La muestra estuvo conformada por personas que asistieron a instituciones médicas del área metropolitana de Bucaramanga. La validación de constructo del instrumento se hizo mediante un muestreo por conveniencia empleando el análisis Rasch. Se analizaron variables sociodemográficas para describir la muestra con el software Stata versión 10.0 y el análisis Rasch con el software Winisteps. Resultados: Se obtuvo una escala con validación de constructo para 25 ítems, realizada en una muestra de 250 personas. La varianza explicada por la escala fue de 41.7%, con índice de separación para las personas de 1.51 y 6.5 para los ítems, y una confiabilidad de 0.69 y 0.98 respectivamente. Fueron establecidos ajustes de buena medida en los MNSQ para Infit entre 0.8 y 1.15, y para Outfit entre 0.59 y 1.6. Conclusiones: Se diseñó una escala confiable y valida, es el primer instrumento con medición unidimensional del estigma hacia la tuberculosis soportado en las propiedades del análisis Rasch según las recomendaciones de Wolfe y Smith. La generalización de este instrumento en diferentes contextos se deberá hacer empíricamente usando la metodología Rasch.
dc.description.abstractenglishIntroduction: Tuberculosis remains a public health problem with a high burden of the disease for the population mainly in middle and low income countries. Factors like the inopportune diagnosis or the adherence of its treatment limit the appropriate control. The magnitude of the stigmas effect on the control is unknown or maybe underestimated because there are no valid instruments to quantify and measure the phenomenon. This hinders the exploration of its effects in endemic áreas of the disease. Objective: Design and validation of a scale to measure the stigma of Tuberculosis in persons with residence in the metropolitan área of Bucaramanga. Methodology: Study of the evaluation of diagnostic technologies with design and validation of an instrument. The sample was taken from people who attended medical institutions in the metropolitan área of Bucaramanga. The construct validation of the instrument was realized through a convenience sample and by applying Rasch analysis. The Socio-demographic variables to describe the sample were analyzed with the Stata software 10.0 and the Rasch analysis was carried out by Winsteps. Results: Through the construct validation process an instrument for 25 items was obtained and applied within a sample of 250 persons. The variance explained by the instrument was 41.7% with a separation index for the persons of 1.51 and 6.5 for the items. The reliability for the persons was 0.69 respectively 0.98 for the items. Appropriate adjustments were made in the MNSQ to Infit were between 0.8 and 1.15 and to Outfit between 0.59 and 1.6. Conclusión: A reliable and valid instrument was designed. This is the first instrument with a unidimensional measurement of the Tuberculosis stigma which is supported by the Rasch analysis according to Wolfe and Smith. The generalization of the instrument in different contexts has to be done empirically with the use of the Rasch methodology.
