Resultados radiológicos y complicaciones de osteosíntesis de fracturas de acetábulo en el hospital universitario de santander

dc.description.abstractLas fracturas de acetábulo son lesiones desafiantes para el cirujano ortopedista debido a las dificultades que conlleva lograr una reducción anatómica y una cadera móvil, funcional y viable. El objetivo de este estudio es evaluar los resultados radiográficos inmediatos y las complicaciones tempranas de las fracturas de acetábulo osteosíntesis en el Hospital Universitario de Santander entre 2012 y 2020. Se realizó un estudio de cohorte retrospectiva que incluyo 54 pacientes consecutivos (42 hombres, 12 mujeres) con una edad promedio de 38.2 años, los cuales fueron tratados quirúrgicamente por un cirujano experimentado en un centro de trauma de alta complejidad entre 2012 y 2020. Para cuantificar el resultado radiográfico, se utilizó la escala de Matta. Se evaluaron las complicaciones intraoperatorias y postoperatorias durante la estancia hospitalaria. se identificaron 54 fracturas y se utilizaron las clasificaciones de Judet y AO. Los patrones más frecuentes fueron las fracturas de la pared posterior (29,6) y pared posterior más hemitransversa posterior (20,4%). De todos los pacientes, la reducción se calificó como anatómica en 70,4%, no anatómica-no perfecta en 25,9% y como pobre en 3,7%. Se encontró un 5,5% de lesión neurológica, 3,7% sufrieron una infección y 1,9% de mortalidad, no se presentaron otras complicaciones durante la estancia hospitalaria. El tratamiento quirúrgico de las fracturas de acetábulo en nuestra institución, en general tiene unas tasas bajas de complicaciones postoperatorias tempranas y unos resultados radiológicos postoperatorios adecuados, lo que permite inferir que el tratamiento quirúrgico es una opción segura y con buenos resultados a corto plazo.
dc.description.abstractenglishAcetabulum fractures are quite challenging injuries for the orthopedic surgeon due to the difficulties involved in achieving anatomical reduction and achieving a mobile, functional and viable hip. The objective of this study is to evaluate the immediate radiographic results and early complications of osteosynthesis acetabulum fractures at Hospital Universitario de Santander between 2012 and 2020. A retrospective cohort study was conducted that included 54 consecutive patients (42 men, 12 women) with an average age of 38.2 years, who were surgically treated by an experienced surgeon in a highly complex trauma center between 2012 and 2020 To quantify the radiographic result, the Matta scale was used. The most frequent patterns were fractures of the posterior wall (29.6) and posterior wall plus posterior hemitransverse (20.4%). Of all the patients, the reduction was classified as anatomic in 70.4%, non-anatomical-not perfect in 25.9% and as poor in 3.7%. 5.5% neurological injury was found, 3.7% suffered an infection and 1.9% mortality, no other complications occurred during the hospital stay. The surgical treatment of acetabulum fractures in our institution generally has low rates of early postoperative complications and adequate postoperative radiological results, which allows us to infer that surgical treatment is a safe option with good short-term results.
