Confiabilidad y validez de la evaluación de la movilidad costal y de la fuerza de los músculos respiratorios en adultos con asma

dc.description.abstractEl Índice de Amplitud Torácica (IAT) y la Valoración Muscular Manual (VMM) son herramientas útiles para evaluar la mecánica ventilatoria en adultos con asma; sin embargo, en la literatura revisada existen pocos estudios que determinan sus propiedades psicométricas. Este proyecto evaluó la confiabilidad y la validez del IAT y de la VMM de los músculos respiratorios, en adultos con asma. Se desarrolló un estudio de pruebas diagnósticas, con muestreo tipo corte transversal; asignación aleatoria a las mediciones realizadas de forma independiente por dos evaluadoras. Se incluyeron variables de movilidad torácica (IAT y cinemática) a nivel axilar, xifoideo y abdominal; de fuerza muscular (VMM y presiones respiratorias estáticas); y variables sociodemográficas, antropométricas y relacionadas con la enfermedad. La confiabilidad del IAT se evaluó por el nivel de acuerdo, con el método de Bland y Altman; y por la reproducibilidad, utilizando el coeficiente de correlación intraclase (CCI2,k). La reproducibilidad de la VMM fue medida utilizando la kappa ponderada. La validez de constructo externa convergente entre el IAT entre y la cinemática, y entre la VMM y las presiones respiratorias, se evaluó con el coeficiente de correlación de Pearson. El nivel de significancia fue p<0,05. Participaron 26 adultos, en fase estable del asma. Para el IAT, la confiabilidad intra y entre-evaluadores estuvo entre aceptable y buena; la reproducibilidad de la VMM, entre sustancial y moderada. En ambos casos, la validez estuvo entre moderada y alta. El IAT y la VMM son mediciones válidas y reproducibles que pueden ser utilizadas para evaluar la movilidad del tórax y la fuerza de los músculos respiratorios en adultos con asma; en el ámbito clínico contribuyen a una valoración objetiva, y en investigación, pueden ser útiles para demostrar los resultados de las intervenciones, disminuyendo la probabilidad de que los resultados sean influenciados por la variabilidad en las mediciones. _______________________
dc.description.abstractenglishThe thoracic amplitude Index (IAT) and the muscle assessment Manual (VMM) are useful tools for evaluating the mechanical ventilation in adults with asthma; however, the studied literature shows few studies that determine its psychometric properties. This project evaluated the reliability and validity of IAT and VMM of the respiratory muscles in adults with asthma. A study of diagnostic tests was developed, with a cross - type sampling section; random measurements were performed independently by two evaluators. Thoracic mobility variables were included (IAT and kinematics) at an auxiliary level, xiphoid and abdominal; and muscle strength variables (VMM and static respiratory pressures); and sociodemographic, anthropometric and disease-related variables. The reliability of the IAT was assessed by the level of agreement with the Bland and Altman method; and by the reproducibility, using the intraclass coefficient correlation (CCI2, k). The reproducibility of the VMM was measured using the weighted kappa. The validity of external converged construct between IAT and the kinematics, and between the VMM and the respiratory pressures, were evaluated with the Pearson correlation coefficient. The level of significance was p <0.05. 26 adults participated; they were at a stable phase of asthma. For the IAT, the reliability intra and between evaluators it was between acceptable and good; the reliability of the GMM, between substantial and moderate. In both cases, the validity was between moderate and high. The IAT and the VMM are valid and reproducible measurements that can be used to evaluate the mobility of the thorax and the strength of the respiratory muscles in adults with asthma; in a clinical setting, they contribute to an objective assessment, and in research, they may be useful to demonstrate the results of interventions, decreasing the probability of outcomes being influenced by variability in the measurements. _______________________
