dc.description.abstract | El Sistema Nervioso se ve comprometido en el 50-70% de los pacientes con infección por VIH-SIDA. Se realizó un estudio de corte transversal que describe un grupo de pacientes con infección VIH-SIDA y compromiso del Sistema Nerviosos Central (SNC), que consultaron al servicio de urgencias de un Hospital de III nivel en el lapso de un año. Fueron 23 pacientes, que representan el 21% de los pacientes con VIH-SIDA que consultaron al servicio de urgencias de este Hospital en el año 2003. A estos pacientes se les realizó una anamnesis médica; examen neurológico; y análisis del líquido cefalorraquídeo (LCR) que incluía: presión de apertura, citoquímico, tinciones de Gram, tinta china, KOH, Ziehl-Neelsen, látex para criptococo, cultivo para gérmenes comunes, cultivo para micobacterias, VDRL, y ADA. Además, Tomografía Axial Computarizada o Resonancia Nuclear Magnética. En algunos de los pacientes se realizó conteo de linfocitos CD4, carga viral e IgG para T. gondii. Los diagnósticos finales fueron: toxoplasmosis cerebral 9 pacientes (39.1%), Criptococosis meníngea 6 pacientes (26.1%), lesión expansiva única del SNC 3 pacientes (13%), lesiones expansivas múltiples del SNC 3 pacientes (13%), meningitis linfocitaria aséptica 1 paciente (4.4%) y Meningitis linfocitaria aséptica asociada a infección aguda por VIH 1 paciente (4.4%). La focalización motora, principalmente hemiparesia, fue muy sugestiva de toxoplasmosis cerebral. La rigidez nucal y el síndrome de hipertensión intracraneana fueron más frecuentes en criptococosis meníngea, estas diferencias tuvieron significancia estadística. El LCR de los pacientes con criptococosis meníngea y toxoplasmosis cerebral fueron normales en el 17% y 25% respectivamente; en los demás predominó hipoglucorraquia leve, con proteinorraquia moderadamente elevada y discreta pleocitosis linfocitaria, siendo ésta última más marcada en la criptococosis meníngea. No se documentaron casos de TBC meníngea. | |
dc.description.abstractenglish | The Nervous System becomes to be compromised in the 50-70% of the patients infected by HIV-AIDS. A study on transversal cut was developed in order to describe the compromise of the Central Nervous System (CNS) in a group of patients infected by HIV-AIDS, who consulted the urgency service of a third level Hospital during a year. 23 patients were treated, representing the 21% of the patients infected by HIV-AIDS that consulted the urgency service of this Hospital in 2003. In these patients the following studies were done: medical anamnesis; neurological examination; and cerebrospinal fluid (CSF) analysis, that involved: aperture pressure, cytochemistry analysis, Gram's stain, India ink, KOH, Ziehl-Neelsen, latex for cryptococcus, culture of common germs, culture of mycobacterium, VDRL, and ADA. Besides, Computarized Axial Thomography or Magnetic Nuclear Resonance. In some patients a count of CD4 lymphocytes, viral charge and IgG for T. gondii were carried out. The final diagnostics were: cerebral toxoplasmosis in 9 patients (39.1%), cryptococcal meningitis 6 patients (26.1%), unique expansive injury of the CNS in 3 patients (13%), multiple expansive injuries of the CNS in 3 patients (13%), aseptic lymphocytarie meningitis 1 patient (4.4%) and aseptic lymphocytarie Meningitis associated to acute infection by HIV in 1 patient (4.4%). The motor focalization, mainly hemiparesia, was very suggestive of cerebral toxoplasmosis. The neck stiff and the syndrome of intracranial hypertension were more frequent in cryptococcal meningitis; these differences had a statistical significance. The CSF of the patients with cryptococcal meningitis and cerebral toxoplasmosis were normal in the 17% and 25% respectively; in the other patients predominated slight hypoglycorachia, with proteinrachia moderately elevated and discrete lymphocytarie pleocytosis, being the last one highest in the cryptococcal meningitis. Cases of meningitis TB were no documented. | |
